There is a hole but it's behind the round thing. You need to feel around behind (point towards you) it and there really is a hole, push and the center round piece pops out.

Once you get all the fasteners around the perimeter seperated by pulling the panel gently away from the door, the chrome piece comes off with the panel by pulling up holding onto the sides of the panel.

Back arm rests, I left mine in - I only gutted the interior to put in new stereo / speakers and did not need to remove those. If I remember correctly it looked like they would come out with the panel but did not try to remove them.
