Shift knob came off while driving!!! (archive)

It was really frightening. I was doing a usual (or so I thought) 1->2 shift when I feel the knob give way and come off completely. I don't understand how this could've happened considering all the stories I hear around here about guys breaking windshields trying to get theirs off...
Anyway, any ideas how to properly reattach it or do I just jam it back on?
