Compared to the stock E39 528 shod with small 15" wheels, I would have thought that 18" wheels fill up the wheel well quite nicely.

What would I be gaining by going to 19" or 20"? Just looks? Would any disadvantages start kicking in at that point?

: 19" or 20" rims also fit on the new 5series. While 20's would be a extreme fit I'm sure that it could be made to work. Since you already have a set of tires and rims for winter why not go for the 19" set up? YOu can have larger tires and they will fill up the wheel well even better than the 18's.
