The third seat belt receptacle probably got tucked under the rear seat. You can remove the seat bottom by pulling up sharply on the bottom front edge of the rear seat behind the driver's and passenger's seats to release the rear seat bottom.

It might be tucked down in that small recess below the fold down armrest (that is where I tuck mine when not in use.

There are 3 parts to the middle belt.

The long belt that pulls out of the package shelf. The small metal clip on the end of this is to be folded back and inserted into the little storage hood further back on the package shelf when the belt is not in use.

When the belt is in use then that small end clip is inserted in a small buckle (which is on the end of a small length of belt) just to the right side of the middle seat.

Then the belt is fastened into a larger fastener (also on a short piece of belt) on the left side of the middle seat.

It is likely that, as Mike said, both these short centre belts have beem left down below the seat.

Need to pull them up then store them in that little recess as mentioned above.
