I think a reasonable definition would be that it is rusted to the point that there is a hole of any size clear through the metal.

Even if there is a pinhole, it is obvious that the hole will grow very quickly in size.

I would think that some bubbling of the paint which can be cleaned free of rust without going through the metal would not qualify as perforation.

However, be sure to poke at the rusted area to make sure it is not a thin skin of rust. If a poke with a screwdriver goes through, it's perforated.

This shouldn't happen with today's corrosion treatments, I would think.
