You will need to use the send/end button (looks like a handset) located to the right of the navigation screen (below the tilt button on newest 7 series) - the send button will then toggle between calls 1 and 2. The end/send button on steering wheel is now also doing voice activation feature so unable to do call waiting from steering wheel.

We all hope that this works. But, I believe someone on this list tried this and terminated the call when pressing this particular button during a call. Will try it and let everyone know of my result tomorrow. One other point, Caller ID does reveal initial incoming calls, but only a beep signals the Call Waiting feature - not the new caller-number coming in.
THIS DOES NOT WORK !. You can NOT answer an incoming call with Call Waiting with the BMW 2001 - 740 series without picking up the handset from the cradle while you are driving, then looking for the "send" button, then pushing it, in order to accept a "call waiting" call. No other button seems to toggle between calls.
This may currently be a violation of the law in certain states which prohibits drivers from using a handset while driving. I believe that certain states have enacted laws which require you to use ONLY a speaker phone while driving in order to provide for safety while driving.
I hope that BMW works with Motorola to come out with a retrofit for this as soon as possible. Otherwise, their customers who have spent $70,000 on a car may be driving in an unsafe driving condition, and may be violating the law in several states, if they are forced to pick up the handset in order to answer an incoming 'call waiting" call.
Probably all of you are calling your dealers to complain about this and maybe someone will bring this to the attention of BMW regarding these brand new 2001-740's.
