My Denon CD changer (same mag as the pioneer) does this when the lens gets dirty. More dust on the lens means a more scattered beam, and less tolerance to bumps and such. Bought a CD lens cleaner (looks like a regular CD with a small cloth tab in the middle of it), and the problem went away.
