typical driving is under 40mph in suburban and some arterial road driving in non peak periods.

Do youi have an automatic or a 5 speed? I would suggest that you could do better than 12 MPG in town. One possible problem that could lead to low fuel milage is the lack of an operational O2 sensor. With out the O2 sensor working the controller will revert to a full rich condition and the fuel injectors timing will cause excess fuel to be injected in the engine. When was the last time you did a good tune up, and check the plugs, wires, etc? Another possible problem could be the temperature sensor. If the resistance reading never falls to a low level as the coolant warms up the controller may keep the injectors in the full rich condition. And what position is your foot on the accelerator? Are you always running the engine up to 4-5K RPMs? That will cause the milage to suffer.
