I've never SEEN an E23 headrest move. I don't think they really do.

Of course, you could remove the seatback, and then you'll see all the workings of the headrest.
You'll have to fiddle with it.


The headrests on my '84 733i still work just fine (driver's and passenger's) - knock on wood.
