I agree with Tom. It appears that your heater core or the pipes that lead to the heater core are leaking. This would explain the coolant inside the car. It also could be the hose connections to the heater core pipes. But prepare for the worse as it probably will be the heater core. It can be changed. You do not have to remove the dash. But it will take some work. I will send some photos for you to look at.
The other problems does sound like a HVAC control problem or a lack of vacuum to the system. That will require a diffent set of trouble shooting options.
But if you do decide to replace the heater core be sure to check all the solenoids and valves as you will have the HVAC covers off to get acess to the core.
