the smoke color will tell you what problems to diagnose. blue smoke indicates burning oil. black smoke indicates burning an excess amount of fuel. since your car puffs the blue stuff, i would suspect that oil is leaking into the cylinder, causing the plugs to foul. this would explain the "miss" for the 2-3 seconds. once the oil is burned off the plugs, the car will then idle smoother. next time pull the plugs and see if you can locate the leak to 1 or a couple of cylinders.
my guess would be the valve seals are slowly going. anyone have any other thoughts??
