shift knob is loose (archive)

The shift knob came loose. I tried hitting down firmly, but no luck. When I pulled the one-piece knob/boot off the shifter, there is a ~3 inch black plastic sleeve that covers the shift rod and is stuck to it. Is this black plastic piece supposed to be fixed to the inside of the knob or is it supposed to be part of the shifter? Is the cheap fix crazy glue? Options?
