The shift knob came loose. I tried hitting down firmly, but no luck. When I pulled the one-piece knob/boot off the shifter, there is a ~3 inch black plastic sleeve that covers the shift rod and is stuck to it. Is this black plastic piece supposed to be fixed to the inside of the knob or is it supposed to be part of the shifter? Is the cheap fix crazy glue? Options?

The entire knob assembly snaps onto the shifter. The plastic piece should have stayed with the knob. If you go the super glue route, I would recommend pulling the plastic sleeve off the shifter stalk then glueing it into the knob. The reason, depending on the viscosity of the glue you use, it may wick between the plastic sleeve and shifter shaft. If this happens you'll never get the knob off the shifter staft without destroying the knob. Personally I would replace the entire knob assembly with a new piece. Hell, you're driving an M5 why be that cheap, it's about 60 bucks for a new M-sport knob and insert of you're choosing.
