See my post below, but here's what has been done:

new coolant temp sensor (tests fine at ECU harness).
rotor, cap, wires, plugs, all <2 years old, inspect/test fine.
Fuel press reg , 2 years old (fuel pressure not tested tho).
idle mixture correct, base idle correct, individual tbodies balanced.
Compression excellent.
O2 sensor < 2 years old, tests fine.
swapped chip back and forth, Jim C - stock.
swapped AFM out of a car which doesn't appear to have this problem. AFM ouput checks fine at ECU.
swapped ECU out of same car.
Valves just adjusted.
K&N cleaned
P44 injector cleaner recently
always premium gas
intake boot new
can't find any air leaks (carb cleaner)
inspected/cleaned ICV (was spotless but cleaned it anyway
tested TPS - fine
Exhaust gas analyzer shows very low HC at idle.

there may be more, but that's all I can think of now!

No mods, other than K&N in stock airbox, Jim C chip.

I leave mine parked outside (no garage) and I can go out and fire it up after sitting for a week at 20-40F temps. All I've done in the last 10k miles is Techron, spark plugs, and drive it *hard*, escpecially if it's for short trips. This gets the engine and oil hot and reducess carbon build up.

What sort of maintenance have you done on your M5 in the last year? Plugs, wires, filters, cap, rotor, injector cleaning, coolant temp sender, ECU chip, fuel pressure regulator, vacuum hoses, etc?
You can eliminate the O2 sensor, because the ECU ignores it until the temp comes up.

What mods do you have?

Let's see if we can solve this one.

I'm fortunate that mine runs like a champ nearly 100% of the time. I DO have a problem with certain brands of gas (Exxon, Sheetz, Citgo, cheap stuff) and the #5 injector sticking shut on cold start. Not fun, but a few taps gets it firing again. I am going to replace the injectors and have the old ones cleaned/blueprinted at RC Engineering. They are the same for the E30 M3, so I'm going to send those off also once I have my spare set.
