I had the same problem, the trucking company that shipped by car tied it down via the sway bars which ripped then right off the frame and took the SLS limit switch with it. I ended up with a cracked switch case and twisted hardware. The internals were fine though.

I would call maximillions, sorry I don't have the number, I recall he has acess to this hard to find and very pricey item. $1,200 from the dealer.
