Bad center bearing. Part is about $40, but the shaft has to come off. Pull it, and check the u-joints. The center bearing is held on by a snap fitting. If the u-joints are good, replace the guibo too (not worth doing this again for awhile).

If the u-joints are bad, don't bother with the centerbearing or guibo. The driveshaft has to be replaced or rebuilt, and if you go the rebuilt, those parts will be included.
