If it is below the throttle bodies then it is a piston related noise.

Invest in an engine stethoscope to pinpoint it further.

Does it occur during driving oor idle or both?

At idle, you could unplug the plug boot one at a time to see if the noise goes away on any cylinder.This would help you pinpoint the cyl.
