I am not the original owner, so can't speak for the record prior to my ownership, but I have not replaced them. My first guess was intake gaskets too. That was why I measured the intake vacuum. It measures a) even side-to-side b) consistent during warm up and c) strong, all characteristics lead me to believe that this is not an intake gasket issue... also, no codes, which are common if the mixture gets too lean.

Kevin - have the intake gaskets been replaced?? The symptoms you describe sound like the result of intake gaskets going sideways. As the engine heats up they expand and reseal themselves.
