Found there are several ways this could have made this catastrophic damage -

1. Over revving the engine. This could break a spring or float a valve.

2. Forgetting to unplug the fuel pump relays before doing a compression test.

3. Unplugging the throttle plug while the engine is revving. I think they were referring to the EML harness or maybe there is a throttle harness and an EML harness next to the diagnostic connector in the engine compartment.

4. Spraying too much cleaner into the engine while your revving it.

5. Unplugging one of the computers and then starting the engine, nothing will tell the injectors on the unplugged side to shut off.

6. Dropping something into the intake manifold (screw, nut, bolt).

7. Something in the oil (like a metal shaving) getting into the valve sleeve and causing the valve to stick open. The piston would come up and hit the valve head and the rest is history.
