When I bought my 1991 535i about a year ago, it had the same problem. The first thing I did was a complete tuneup, including cap and rotor, plugs, filters and valve adjustment. That helped some. I've also been running premium Chevron gas with Techron. Techron is an additive that is supposed to clean up the injectors and such. My car does idle much better now, however, it's still not as smooth as the 5.0 H.O. Mustang engine I put in my Thunderbird. Personally, I think the "big six" engines just have a slightly rough idle. However, in relation to how it runs the rest of the time, I'm willing to make the trade off.

Hope this helps some.
