I had the same problem, and after trial and error I finally got to the root of the problem.

Check the bypass hose, the sensor and the thermostat. The thermostat may be sticking, and the "bypass hose" may need cleaning/replacing.

If that doesn't fix it, check the water pump and the fan clutch.
