Haven't heard of this happening much on E34s but you never know how someone tried to tighten the plug previously. I use a new plug each time I do a coolant change.

I'm wondering whether the radiator didn't drain because: 1) the cap was still on the expansion tank, thereby holding a vacuum or 2) there's scale in the radiator plugging up the port. Try to flush out the cooling system if you want to pursue this problem. Remember, these cars need careful bleeding of the cooling system.
