I've often wondered the same myself. When the wife and I got married, she had recently been given (!?!) her parents old E28 '86 528e, which they had owned since it had 9k miles on it. They had traded up to Ginger, an E34 '95 525ia, in 1996, which they bought with 13k on the clock. The wife drove the E28 until March of '99, when the in-laws decided they wanted an E39. We bought the E34 from them for trade-in at 52k miles. I perform oil changes and such, and have done O2 sensor, brakes, radiator, etc. myself. I let a good local indy do the Inspection I and II. The car ran with dino juice until 77k, then I switched to Mobil 1 with no apparent ill effects.
Since we got the car, we've spent just a shade over $3100 on the car, including oil changes, inspections, repair parts, and indy labor; my labor is free (haha). So it's really broken down to about 10 cents per mile maintenance cost, doing 80% of my own work. Maybe figure 15 to 20 cents per mile if the dealer services it for you, depending on how hard you are on a car. My wife drives this car 95% of the time, so she's pretty easy on it.
