I have the exact same symptoms, not sure what's causing it and I haven't tried to chase it much. I'm eager to see what people think.

Experiencing rhythmic idling only when cold, up and down between 600 - 1000 rpm, feels like the ignition is lost for a fraction of second. A lot worse at cold mornings.
Press on accel. pedal give a cloud of gray smoke (not blue like from oil), very strong smell of unburned fuel from exhaust, like only the fuel is coming out. Very hot tail pipes after 20 min. of driving.
Somethimes long cranking times when left inclined and warm (not hot) engine. Correlated problems?

Changed spark plugs, fuel filter, air filter, run several fuel injector cleaners, cleaned whole AFM assembly, cleaned idle control valve,.....nothing helps.
