There's a whole string of possibilities. Firstly,as far as I know, all M5's have erratic idle, due to the hot cam and big valves. Mine gets worse when stuck in traffic, and I think it's because it just can't burn the fuel efficiently enough at idle. It sometimes gets so bad that I hear pretty loud explosions in the exhaust system (when idling), caused by unburned fuel.

Secondly, it could be sparkplugs, valve timing or leaking, filthy injectors, etc.

The O2 and other sensors and elctronics can easily be checked by the BMW dealerwhen hooked up to their computer.
