They usually don't cover "pre-existing" problems.

Example:
Let's say your transmission fails.
The warranty company could say that it failed due to a problem which existed before you bought the warranty, and therefore is not covered.
How can you prove otherwise?

Personally, I think any warranty you have to pay for is a waste of money.
