Place the shift lever into 2nd gear. Place your left hand over the front of the knob. Place your right hand at the back of the knob. Similar to a golf club, but not exactly the same. With your feet on the floor underneath the clutch and brake pedals, push up (off) straightening your body and the knob will come right off. This way, the knob is heading towards your gut instead of your head. It never hits your gut because your fore arms get in the way. This method is also much easier on the wires if you happen to have an illuminated shift knob.
