According to both the NHTSA and the Canadian equivalent ALL cases were due to driver error, related to the fact that the brake and accelerator pedals are fairly close together. In fact, the rate of unintented acceleration incidents in the 5000 was only slightly higher than that of same year Buicks. The Audi that 60 minutes demoed had been extensively modified to "simulate" the problem. Audi should have sued 60 minutes - they did successfully sue one of the organizations that was trying to force them to recall all their cars.
