You can probably use the "Optional Image URL" field in the followup form. I typically just paste in the HTML for the picture. I'd post the HTML itself, but I'm not sure how to get this form to leave it "as is" instead of treating it as valid HTML.

You can see what the HTML looks like by using the "View Source" option in your browser. Scroll down until you see the "img" tag. There will be a "less than" symbol just before the word "img", and everything between this and the "greater than" symbol makes up the image display code. The quoted text after the "src" keyword is the URL for the image, and this needs to be a valid URL accessible by anonymous users.

If you need a place to host the pictures, photopoint.com is a popular place. It's easy to use and they give you space for free, up to 1M I think. Upload your picture there, view it in a browser, and then copy the URL. I typically get the URL from the image properties (use right mouse menu to get to this) as this is the image only, not any other page info.
