I observed a simular behavior on one occassion. In thinking about what I was doing at the time when the error was thrown, this was the only action I could attribute. I might have aggressively shifted gears while the vehicles momentum was high given the mass(5k lbs). Since, I've been more conservative while shifting. Either at a complete stop or mindful of the mass versus momentum.
