I used to be "haphazard" about the use of my seat belts too. A good friend of mine opened my eyes one night when he told me something he had noticed in the many years of him cutting people out of automobile wrecks as an EMT with different Rescue Squads...
He had cut MANY dead people out of wrecks that were NOT wearing seatbelts, and had cut survivors out of nearly identical wrecks that were wearing their seatbelts. They were usually bruised, cut, maybe some broken bones, but they did survive and recover. And he had proof to show me.....
He followed this up with a home slide show of accident scenes and graphic pictures of the corpses or survivors taken from them.
We were both 3 sheets to the wind at the time, and this is how he talked me out of driving home. It worked, and I still remember the pictures after over 20 years along with the lesson of wearing seatbelts (not to mention don't drink and drive).
