Actually, you use the ball of your foot. You have to brake with the left side of your right foot. Then while applying brake pressure, roll your foot over to the right and start applying pressure to the accellerator. Then when you have the revs up, you chamge gears.

I have to admit, in the driving I attended, I was never able to master this technique very well.
It definitely takes some practice.
