1. Why did this happen? Besides my sloppy shifting, isn't there something that prevents this?

2. Did I do any permanant damage to my car? It drives fine but I wonder I just took 10,000 miles off the life of my clutch...

3. Has this happened to anyone else?

Unfortunately this is a pretty common occurrence with BMWs, particularly M3s. Many an M3er has blown an engine because it is waaay too easy to accidentally shift into the wrong gear.

So, the good news is that your engine sounds like it is running ok. The bad news is that you may have done some damage and should get the engine checked out. More bad news is that the next time your local BMW dealership hooks your car up to their computer, it will report the overrev condition to them--it is stored in the car's computer's memory.

You don't need to worry about your clutch--you need to worry about your engine as you may have caused damage to the valves.
